Annual fee cannot be used with CareCredit or third-party financing
3% service charge applies to credit card payments
Plan is non-refundable if not used
Plan renews annually and is paid in full
No monthly billing to bank or credit card

Program Limitations & Exclusions:

Cannot be used at other dental offices or specialists
Treatments referred to specialists are not included
Does not cover hospitalization or emergency room visits
Does not cover trauma-related dental care
Does not cover services eligible under Workers’ Comp
Includes a 1-year warranty for fracture or breakage
